Medicare – Patient Care Management UI
Medicare is a web system that presents information about healthcare services, health resources, and care centers. It also includes an appointment request form interface for patients.
About This Solution
Medicare is designed to help patients find healthcare information and services easily. From the home page, users can explore healthcare services, specialist care centers, locations, and health articles.
Clicking Request Appointment in the navigation area opens a modal form where patients can enter their name, date of birth, email address, phone number, preferred doctor, hospital, specialty, reference code, gender, appointment date, and address.
The appointment form is currently a UI preview only. It does not send appointment requests to a backend or save information to a database.
